How can I file a written complaint with a social services or health service?

If you have a complaint regarding a social or health service, it is advisable to try to speak with the person who committed the discrimination or rights violation, or with a person responsible for the service or entity. If this is not possible or has not yielded positive results, you can submit the complaint in writing or, more formally, using a complaint form. In both cases, it is important that the person filing the complaint keeps a copy stamped by the center or service confirming receipt.

One way to organize the ideas in the written complaint is with the following outline:

  1. Begin by explaining the purpose of the writing (to inform, to suggest, to protest...) and the fact that motivated the complaint (the action, attitude or situation), specifying the place and day in which it occurred.
  2. Identify yourself (as a person with a mental health problem, a family member, a professional, a person sensitive to mental health…) and explain how the discriminatory or violation has bothered and affected you.
  3. Indicate how this fact violates the rights enshrined in the International Convention on the Rights of Persons with Disabilities (if known, specifying the specific article) and in other regulations that may be known.
  4. Request that the complaint be addressed and that the corrective measures deemed appropriate be taken.
  5. Sign the document, including your identification details (full name, ID or passport number, address, place and date of the document).
