Reconecta, a tool to apply the recovery model in mental health
In mental health services there is a diversity of shared Individualized Work Plans (PTIC) and the person may have to start a new plan in each service where they are cared for, which reduces continuity in their therapeutic process. At the same time, these plans are often designed in a unidirectional way by the professional teams of each service and relegate the person being cared for to a more passive role.
In order to respond to this situation, the Individualized Services Plan (PSI) program of the Sant Joan de Déu Health Park has created Reconecta, a dynamic tool framed within the recovery model and person-centered intervention, which has the following objectives:
- Offer complementary support to optimize care for the person .
- Make the Work Plan more accessible for each person so that they can participate more actively in its design and monitoring.
- Facilitate communication between services and cross-functional monitoring of the recovery process of the person being treated in a more effective way.
Reconecta is committed to enhancing the person's decision-making capacity and transforming the way they are supported, based on a change in roles, attitudes and values. This is why the therapeutic bond between the person being treated and the accompanying professional is of paramount importance. This is a methodology that is aimed at all people treated in the mental health network who have a link with a reference person or professional, and who voluntarily want to work on their PTIC with this format.
- Therapeutic bond. There must be a prior bond between the person being treated and the professional who presents and offers this new proposal. They can be professionals of diverse profiles, depending on each service, although so far they tend to be PSIs. This bond is based on a relationship of support, trust and accompaniment without judgment or imposition.
- Voluntariness. When the person actively participates in the changes and accepts the approaches of the reference people who accompany them, their recovery process will be enhanced.
Reconecta enables the person being treated to participate more actively in the development of their Work Plan and optimizes communication between the various services that assist them, with the ultimate objective of enhancing the mental health recovery model.
Within this framework of the person-companion relationship, and in the different spaces of visitation, accompaniment and professional support, the person defines the objectives they want to achieve in the PTIC, with their own language and within the deadlines that each person needs. The result is, therefore, a Work Plan made with and for the person being cared for, which includes:
- Their objectives , broken down into scheduled activities and each linked to support services or people. These objectives can encompass all areas of the person and be linked to housing, training and work, procedures, health, transportation, money and savings, personal relationships and/or leisure time.
- An agenda , which includes those significant events for the person at all levels (medical, therapeutic, work, personal...).
If the person so wishes, Reconecta allows them to work on and incorporate into the document a Crisis Plan (guidance in the event that situations that anticipate a crisis arise) and their Advance Decision Plan (PDA).
The Shared Individualized Work Plan developed with Reconecta has a very visual and first-person presentation , written in your own words, and will accompany you in all the services where you are attended to. In this way, the different devices can always know what the person's wishes are. On the other hand, in the successive meetings with the person, moments can be established to review the objectives and update the agenda and the Crisis Plan.
Currently, Reconecta is already being piloted in some mental health services of the Sant Joan de Déu Health Park (CSMA, therapeutic community and adult Day Hospital), with the plan to gradually implement it in the rest of the institution's services, both hospital and community.
